Before your Appointment

1. Download and fill out the patient registration document by clicking the button below. You should have these documents completed and sent before your appointment. Due to the current pandemic, please send back the forms one of these ways:

2. Call us to book your appointment.

3. When you arrive please make sure you are wearing a mask. Face masks are required to be worn inside the office.

4. If possible, please include a copy of your insurance card front and back and your photo ID.

5.. On the day of your appointment please call the office at 256-240-7272 once you are in the parking lot and we will give you directions for your visit.

Mon - Thurs: 8am - 5pm

Friday: 8am - 12pm By Appointment Only

Everyday: 12pm-1pm Closed for Lunch

Insurance

In order to be seen in the office, you need to bring a copy of your current insurance information or your insurance card, plus a valid ID.

If you have a type of insurance that has a referral, make sure you have discussed this with your referring physician, prior to arrival.
